We currently have an exciting opportunity for you to bring your excellent work ethic as a dedicated Social Worker to our new Urgent Primary Care Centre located in Surrey, British Columbia. The first of its kind in the province, our state-of-the-art Urgent Primary Care Centre will provide primary care to a range of vulnerable and complex populations including: families, complex frail adults, and people with mental health and substance challenges. Services will be provided by appointment and through walk-in traffic, resulting in a dynamic environment, requiring a high standard of flexibility from all team members.

Build on your education & career experience as you:

Work within a high functioning inter-professional and collaborative team to provide assessments, short-term interventions/support, and partner with community agencies to meet client and family needs. In addition, you will:

  • Deliver comprehensive therapeutic services by exploring the psychosocial response to identify problems, personal concerns, treatment objectives;
  • Acts as a clinical resource for clients and their families by providing information regarding community resources and facilitating referrals;
  • Develop, lead, and deliver in-services, presentations, psycho-educational and support groups and training by collaborating with members of the interdisciplinary team to research relevant content, conduct literature searches, and plan and lead seminars.
  • Initiate, develop, and carry out research activities, program planning, and development relevant to the needs of the patient, families, hospital and community.
To join our team you have:
  • Master's degree of Social Work
  • Current full registration with the BC College of Social Workers
  • Two (2) years' recent related experience in a complex health care environment such as a community or home health setting
  • Valid BC Driver's License and access to personal vehicle for business-related purposes.
